Registration forms need to be completed in full. The details need to be accurate as this will assist the practice in obtaining your old medical records. Please include your previous address including postcode, also your previous GP and practice name.
If you are taking any regular medication we ask that you obtain a repeat slip from your previous GP surgery as this will ensure we are giving you the right medication.
It is important to include the main language that you speak and whether you can understand English, this will assist the practice when seeing you for consultation.
To ensure that you can register at this practice your address needs to fall within our practice boundary. The boundary covers Nuneaton Town Centre, St Nicolas Park, and some parts of Bermuda, Stockingford, Weddington and Whitestone. Please contact a member of staff at the practice who will be able to confirm that your address is within our boundary.
If it is confirmed you are within our practice boundary, you are welcome to come to the practice and complete a new patient questionnaire, one for each family member who wishes to join the practice.
